Compared with whom? Addressing the prognostic value of ambulatory blood pressure categories.

چکیده
and referred cohorts of hypertensive patients,5,6 ambulatory blood pressure (BP) proved superior to clinic BP for prediction of the risk of major cardiovascular events and mortality. These studies analyzed ambulatory BP as a continuous variable.1–6 However, for risk stratification and management of patients, clinicians need operational thresholds, which, by definition, are somewhat arbitrary. Indeed, several clinical categories based on ambulatory BP have been proposed over the last 2 decades well before the evidence of the prognostic superiority of ambulatory over clinic BP as continuous variables became clear. “White-coat” hypertension (WCH), “masked” hypertension, “nondipping,” “overdipping,” and “early morning rise” BP patterns are examples of clinical categories of which the prognostic value has been investigated in outcome-based studies.

Comparative prognostic role of nighttime blood pressure and nondipping profile on renal outcomes.
Different studies have addressed the predictive role of nighttime hemodynamics on cardiovascular and renal outcomes, although nocturnal blood pressure (BP) phenotypes (i.e. nondipping pattern and absolute nocturnal BP) have been found to be predictive of worse health outcomes. Furthermore, differences in both examined populations - ranging from healthy and younger subjects to those with overt c...

“Prognostic Value of Ambulatory and Home Blood Pressures Compared With Office Blood Pressure in the General Population” To the Editor: We read with interest the paper by Sega et al regarding the prognostic value of ambulatory, home, and office blood pressure in the PAMELA population.1 However, we find that the main conclusions of the report may be driven by the lack of adjustment for confounder...
متن کاملPrognostic accuracy of day versus night ambulatory blood pressure: a cohort study.
BACKGROUND Few studies have formally compared the predictive value of the blood pressure at night over and beyond the daytime value. We investigated the prognostic significance of the ambulatory blood pressure during night and day and of the night-to-day blood pressure ratio. METHODS We did 24-h blood pressure monitoring in 7458 people (mean age 56.8 years [SD 13.9]) enrolled in prospective p...
متن کاملEffect of aging on the prognostic significance of ambulatory systolic, diastolic, and pulse pressure in essential hypertension.
BACKGROUND This study compared the relative prognostic significance of 24 hour intra-arterial ambulatory systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), and pulse pressure (PP) parameters in middle-aged versus elderly hypertensives. METHODS AND RESULTS A total of 546 subjects aged <60 years and 142 subjects aged >/=60 years who had undergone baseli...
متن کاملPrognostic value of ambulatory blood pressure : current evidence and clinical implications.
This article is a critical review of the available evidence on the prognostic value of ambulatory blood pressure (ABP). Several event-based cohort studies have shown that ABP improves cardiovascular risk stratification over and beyond traditional risk factors, including office BP. Most of these studies have been conducted in subjects with essential hypertension who were untreated at the time of...
